Caregivers & Direct Support Professionals
in New York

Caregivers and direct support professionals (DSPs) provide hands-on, person-centered care that enables individuals with disabilities to live with dignity, safety, and as much independence as possible — in their homes and communities.

What Caregivers & Direct Support Professionals Do

Caregivers and DSPs provide daily living assistance, respite care, community integration support, and skill-building for individuals with intellectual, developmental, physical, and behavioral disabilities.

Personal Care Assistance

Helping with daily living activities including bathing, dressing, grooming, feeding, and mobility for individuals with disabilities or chronic conditions.

In-Home Support

Providing companionship, supervision, and hands-on support in the home environment so individuals can remain safely at home rather than in institutional care.

Respite Care

Giving family caregivers a temporary break by stepping in to provide care, reducing burnout and supporting the long-term sustainability of family-based care.

Medication Reminders

Assisting with medication schedules, reminders, and tracking to ensure individuals take the right medications at the right times.

Community Integration

Supporting individuals in accessing community activities, social events, appointments, and recreational programs to promote inclusion and quality of life.

Skill Building

Working with individuals on daily living skills such as cooking, cleaning, budgeting, and self-care to promote independence and self-sufficiency.

Emotional Support

Providing companionship, conversation, and emotional presence for individuals who may be isolated, anxious, or in need of consistent social connection.

Adaptive Support

Assisting with the use of assistive technology, adaptive equipment, and communication devices to maximize independence and participation.

Private Caregiver vs. Agency Care

Families have two primary options when hiring a caregiver: working directly with an individual (private hire) or going through a home care agency. Each approach has distinct advantages depending on your family's needs, budget, and how much administrative responsibility you want to take on.

Private Caregiver

A private caregiver is hired directly by the family, either through word of mouth, an online platform, or a referral. The family acts as the employer and manages the relationship directly.

  • Greater flexibility — you set the schedule, tasks, and expectations directly
  • Often lower hourly cost since there is no agency markup
  • Consistent one-on-one relationship with the same caregiver
  • More control over who enters your home and how care is delivered
  • Easier to accommodate last-minute schedule changes
  • Caregiver can be matched to specific personality, language, or cultural preferences

Things to consider

You are responsible for payroll taxes, background checks, and finding backup coverage. If the caregiver leaves, you must recruit again from scratch.

Agency Care

A home care agency employs caregivers and places them with families. The agency handles hiring, training, payroll, and scheduling — the family pays the agency, not the caregiver directly.

  • Agency handles payroll taxes, workers' compensation, and liability insurance
  • Backup coverage when your regular caregiver is sick or unavailable
  • Caregivers are pre-screened, background-checked, and trained by the agency
  • Supervision and quality oversight built into the service
  • Easier to start quickly — no need to recruit or interview independently
  • Agency manages scheduling, HR issues, and caregiver replacements

Things to consider

Agency rates are typically 20–40% higher than private hire due to overhead. You may have less control over which specific caregiver is assigned to your family.
